The next image follows the basic procedure of using two levels of gray. The difference is a slight "modeling" of light and shadow by using the Burn and Dodge tool with the grays themselves as selection sets. You can do this in one pass if you're really confident in your skill level, but it's easier to use the Selection Channel button to make channels for each level of gray. That way you can still change your mind, or fix mistakes easily. Adjust the Exposure level in the tools to something low, like below 20%, to add the effect in a more controlled manner.

You can also use other Photoshop filters to add textures and other effects to the picture, but use them sparingly; Image artwork tends to use the various gradient tools more than this technique. Make sure you have a clear idea in your head of the various shapes and volumes in the picture, as well as where the light sources in the scene are located -- then add highlights and shadows accordingly. In this example, small areas in the woman's blouse were masked off with the Lasso, so I could do spot shading by using the Airbrush tool set to light gray.
